Fusion Accidents, known as Mutations in the Last Bible series, are a recurring game mechanic in the Megami Tensei franchise. Fusion Accidents are unexpected outcomes that randomly occur during Fusion, most often resulting in a different Demon or Persona than the one intended.

Fusion Accidents are a random occurrence that may occur during Fusion in some Megami Tensei games. When a Fusion Accident occurs, the expected outcome does not occur and the original Demon or Persona components are lost. This most often results in an entirely different fused entity than the intended result, one that may be significantly weaker or more powerful. Some Demons of certain Races can only ever be obtained as allies from Fusion Accidents.

Shin Megami Tensei III: Nocturne

In Shin Megami Tensei III: Nocturne, fusion accidents give a different demon than what is said during the fusion process. The fusion accidents are more likely to happen when the moon is fullest, and least likely to happen when it is darkest.

Persona 4

In Persona 4 and Persona 4 Golden, when fusion accidents happen, the Velvet Room inhabitants will have question mark icons over their heads, and the player will not get the Persona they were trying to fuse. There is a 1/64 chance of triggering a fusion accident when fusing two Personas, and 1/32 chance of triggering when fusing three Personas. There are also two types of accidents that can occur. The normal fusion accident will result in a Persona of any arcana other than the Fool, 10 levels under the protagonist. The wild card fusion accident will result in a Fool Persona that is 3 levels over the protagonist.

Persona 5 Royal

Accidents can occur at any time during fusion, though the odds are typically very low. Regardless of the outcome, the Personas involved in the accident are replaced by the new result. If an accident occurs when fusing normal-strength Persona, the result is a random Persona that inherits the prior-chosen skills with no Arcana Burst experience boost and there is no interruption to fusion.

During a Fusion Alarm, all activities within the Velvet Room have greater effectiveness, and the chance of an accident greatly increases for all options but Lockdown. Purchasing Persona from the Compendium registry also increases the likelihood of an accident, including for Group Guillotine special fusions. If no Persona are purchased beforehand, the first action during an alarm will have no chance for an accident and will yield a stronger-than-usual outcome. The affected Persona's name will be displayed in yellow instead of white.

Further actions in a different activity have some chance of accident, while an immediate repeat of the same activity or any action while using a yellow-named Persona are nearly guaranteed to trigger an accident. When an accident occurs while using a strengthened Persona during a Fusion Alarm, the alarm ends and the protagonist must re-enter the Velvet Room to continue further fusion.

Fusion (Guillotine)

If a strengthened Persona causes an accident during the alarm, the resulting Persona will be randomized and have a full spread of skills pulled randomly from a set pool of configurations. There are eleven configuration results for fusing a normal Persona with a strengthened persona, and fourteen set configurations for fusing a strengthened Persona with another strengthened Persona.

Sacrificing (Gallows)

When a Fusion Accident occurs during an Alarm while using the gallows, the benefiting Persona will gain no experience and instead receive a randomized distribution of stat points. The benefiting Persona will gain 5 stat points from a normal Persona sacrifice, 10 stat points if a strengthened persona was the sacrifice; and 15 stat points if both the benefiting and the sacrificed Persona are in the strengthened state. If multiple alarms occur in one visit to the Metaverse and the benefiting persona is registered to the Compendium, removed from the protagonist's stock via fusion/dismissal, and then purchased again from the Compendium, the stat benefit from an alarm accident may be applied multiple times to the same Persona for rapid stat growth.

Itemization (Electric Chair)

Some Skill Cards are only possible to obtain via fusion accident during a fusion alarm. Using a strengthened Persona in the electric chair can result in an element booster or element resistance card based on the following parameters:

  • Element resistance cards are based on the Persona's resistance types (e.g., a Persona with a Repel resistance can itemize to a Repel [Element] skill card, or Drain resistance to a Drain [Element] skill card, while a Persona with both Repel and Drain for different elements may itemize to either Repel or Drain cards).
  • Element resistance cards and Boost/Amp cards are not possible for any element Weakness a Persona has.
  • Dodge [Element] and Evade [Element] are solely possible based on a Persona's Weaknesses to any elements, except Dodge Phys or Evade Phys, which are always possible.
  • Adding an element resistance via an accessory or skill card is taken into account for the pool of skill card outcomes.
  • Dodge/Evade cards and Boost/Amp cards are more likely from low-level and mid-level Personas (<70), while Repel/Null/Drain/Resist cards are more common from higher level (70+) Personas.

In Persona 5 Royal there are no Gun or Almighty resistance/evasion skills, or Phys booster skills. Almighty Boost and Almighty Amp exist as skills but cannot be obtained through skill cards. Some elemental resistance or booster skill cards can also be obtained via other means, but many are exclusive to itemization accidents with a strengthened Persona.

If a standard Persona is itemized and a Fusion Alarm Accident occurs, the result is typically incense with a strength determined by the level of the Persona itemized. However, a rare result of a skill card or accessory may also occur. The results are pulled from the following pool:
