When the location is first entered, it cannot be left until the party enters Room 302 on 3F. There are no encounters up to this point.
Following some scenes, the layout of the location changes, and encounters start appearing. A scripted fight happens, and the party gains access to their initial Personas: Seimen Kongou for the protagonist, Ogun for Mark, Vesta for Yukino, and Aizen Myouou for Nanjo. A level-up will automatically happen upon victory, and in the PlayStation version, a Scalpel and Mop are obtained at the end of the battle.
The party starts from the 2F Lobby and must reach the 1F Lobby. On 1F, in the north-western room, a doctor can heal the party for free.
When the party reaches the 1F Lobby, a nurse is seen stuck underneath a vending machine. If the protagonist interacts with her and chooses "Help her" when Yukino asks for help, the amount of totem rewarded in an event at Alaya Cavern won't be decreased.
Regardless of the choice made, Elly arrives in 1F Lobby, and a scripted battle happens, where she awakens to her Persona Nike. A level-up automatically occurs at the end of the fight, and Elly joins the party. The party can leave the dungeon from this point on. The dungeon can be re-entered up until Alaya Shrine is visited.

Megami Ibunroku Persona
The protagonist, Mark, Nanjo and Yukino first visits Mikage Hospital after Saeko Takami, the group's homeroom teacher, asks them to get a check-up after being found unconscious in the 2F empty room at St. Hermelin High School. The group uses this occasion to also visit Maki Sonomura, a classmate who has been hospitalized for over a year.
In the lobby, while waiting for their check-up, Nanjo suggest to go visit Maki first, to which Mark indicates that her room is Room 302. Nanjo's butler Yamaoka can be seen hiding behind a vending machine. If spoken to, he will ask the protagonist to not disclose his presence to his master.
In Room 302, the party meets Maki who just finished an examination with a doctor. She is grateful to the group for coming to visit her, and is thankful to Mark for bringing her friends to her. If the protagonist previously talked to Yuko Himeno and accepted her request, Yukino will ask Maki if her friend could keep for a bit longer a book titled "Gate to Paradise", to which she agrees since she has memorized it.
Maki says she has been feeling better ever since she has been dreaming of a really nice man, who feels like a father. When Yukino asks if that man is like the doctor she'd just met, she exclaims her dislike for doctors. Mark tries to change the subject, asking about Maki's mother Setsuko Sonomura, but she gets angry, saying she is not her mother due to not attending to her daughter.
As Maki starts to calm down, her condition suddenly worsens, and she is quickly rushed to the ICU. The party waits outside, worried for her, however a strong earthquake starts shaking the hospital. When the earthquake subsides, Mark rushes to the doors of the ICU, only to find that the door now leads into a wall. A scream is heard downstairs, and the party runs there.
When the group arrives in the 2F Lobby, they discover a group of deceased patients turned into zombies, which attacked a nearby nurse, who only managed to escape thanks to the help of Yamaoka, who is now laying mortally wounded on the ground. Upon seeing this, Nanjo loses his composure, and the group is attacked by the demons.
After a zombie misses the protagonist, a Persona named Seimen Kongou manifests itself from the protagonist and defeats one of the demons. The rest of the group discover their Persona summoning abilities as Mark summons Ogun, Yukino summons Vesta and Nanjo summons Aizen Myouou, and the group of demon is quickly wiped.
The battle ended, Nanjo goes to Yamaoka's side, begging for his butler to stay alive, however it is too late. Before dying, he makes Nanjo hold a promise: to become the No. 1 man in Japan, and finally draws his last breath. While Nanjo mourns Yamaoka's death, the party discusses about what their Persona said, concluding that they are not hostile and will help them, and eventually leave the lobby.
When the party arrives at the 1F Lobby after exploring the distorted hospital, they come across a nurse stuck under a vending machine. Yukino tries to help the nurse, but Mark notices demons coming towards the lobby, and Nanjo tells the party to escape the building. The protagonist can choose to either help Yukino rescue the nurse, or follow Nanjo's instructions.
Regardless of the choice, the demons arrive in the lobby, however Elly appears at the scene, and the group is attacked. While Nanjo asks her to stay back, Elly ensures she is a capable fencer, and summons her Persona Nike to defeat the enemies. After the battle, and once Elly comes back to her senses, she explains that just like the party, ever since she played the Persona game, she has dreamt of the butterfly, and that this may be the cause behind their Persona summoning ability.
Elly also tells the party that she came to the hospital to warn them that demons have begun appearing all over Mikage-cho with the exception of the school, and that no one can leave the city. But before heading back to school, she remembers that she found Maki's mother injured at the Alaya Shrine. The group finally leaves the hospital and make their way to the shrine.
Mikage Hospital appears during the SEBEC Route's bad ending. In the PlayStation version, a cutscene shows Philemon's butterfly enter Maki's empty hospital room through the window, and leaving shortly after.
