After the Hero is teleported out of Tokyo by the Heroine to protect him from the ICBMs about to hit the city, he is awoken from his comatose state by En no Ozuno, who introduces himself and his attendents, Goki and Zenki, and then orders the Hero to find his companions Law Hero and Chaos Hero, who were also transported to the realm they currently found themselves in. Exploring the area, rooms where illusions of En no Ozuno are present can be found where he will ask the Hero ideological questions.
When the party reunites and returns to En no Ozuno, he explains that they are in the Diamond Realm and tells them about the desolate state Tokyo has currently found itself in. Seeing their resolve to return to Tokyo, En no Ozuno promises to send them back if they do a set of tasks for him, including fetching some Soma for him twice and sending the Hero to go confront some Phantoms in another area of the labyrinth. Once these tasks are completed, En no Ozuno permits them to leave and sends them on their way.
Shin Megami Tensei II
En no Ozuno solely appears in the post-game dungeon, the Diamond Realm. He will greet Aleph's party upon their arrival to the labyrinth and will send them back to the Underground when they desire it.
Shin Megami Tensei IV: Apocalypse
En no Ozuno will appear as the final Fiend on the bottom floor of Twisted Tokyo if the "Depths of Twisted Tokyo" DLC was purchased.
He also appears in the "Messiahs in the Diamond Realm", where he will awaken Nanashi and direct him to Stephen in order to enlighten him on the details of his request. Any Messiahs Nanashi encounters will go to En no Ozuno, and when all are gathered, En no Ozuno will unlock the way to go meet Stephen once more.
Trivia

- In Devil Summoner: Raidou Kuzunoha vs. the Soulless Army, Hitokotonusi references the legend of En no Ozuno after being defeated by Raidou.[2]
