Teppei Kobayashi and Lotus Juice wrote extended lyrics for the 4-minute-long arrangement on Never More: Reincarnation: Persona 4.

The lyrics were included in the Persona 4 Original Soundtrack booklet. The extended version of the lyrics were included in the Never More: Reincarnation: Persona 4 booklet. The revised Revival lyrics were shown in a motion video that premiered during a Persona 4 music stream for the 30th anniversary.

"Reach Out to the Truth" is the battle background music for normal encounters with Shadows. The song starts off with vocals. In the First Battle version used for the tutorial fight, the vocals are delayed and it instead starts off with the guitar.

"Reach Out to the Truth" is used as the battle theme for normal encounters when the player has the advantage. When they do not, "Time To Make History" plays instead.
