The old woman appears exclusively during Tohru Adachi's Social Link. She lives on her own and dotes on Adachi because she has a son who is also named Tohru.

During rank 1 of Adachi's Social Link, he hides behind the protagonist when the old woman gets off the elevator at Junes in hopes of not being spotted by her. The older woman also has a son named Tohru and she visits Adachi at the police station due to him having the same given name as her son. She acts like Adachi's mother, calling him Tohru-chan, bringing him nimono with hard lotus root, and trying to help Adachi find a wife by giving him photos of women from around Inaba and suggesting that he ask them out. On Adachi's rank 6 event, the woman's son named Tohru has come to Inaba to visit her. When she sees Adachi and the protagonist, she calls her son Tohru-chan and addresses Adachi as Detective. As the woman and her son walk away, she talks about making nimono with hard lotus root for her real son, and Adachi comments that it didn't have to be him.

The old woman appears in episode 6 which adapts Adachi's Social Link from the game. In addition to their interactions from the game, she is shown bringing Adachi pots of nimono at his apartment. He never eats any of it and ends up throwing away all of the food that her and Yu Narukami made for him.
