Kayama is always found in the infirmary and will heal the wounds of the Protagonist and their partner for free each time they visit. The first time the Protagonist visits the infirmary, she is confused about what is happening to the school, but nevertheless gives them 6 ointments and free healing.
When the rest of the school is forced to work in the mines in the World of Sloth, Kayama curiously is exempt from the harsh treatment and remains in the infirmary. Later, once everyone has disappeared from the school after the seal is broken, Kayama is again exempt from the same fate and will comment on the eerie situation. On Reiko's route, when Hazama claims that the school is empty because he does not need anyone, Reiko points out his hypocrisy by bringing up Kayama's presence still in the school, making Hazama flustered.
In one of the flashbacks of Hazama's life found in the Psychic World, Hazama is on his knees hugging Kayama, distressed over the lack of love in his life. He begs for her to comfort him, however, she pushes him off and admonishes him, pointing out they cannot cross the line over their student-teacher relationship.
In Yumi's ending, Kayama thanks Yumi and the Protagonist for saving the school if they visit the infirmary. In Reiko's ending, Kayama instead says that Hazama bore his heart to her and expresses regret for not listening to him more seriously.
