In Persona 4 and Persona 4 Golden, Sayoko is the Devil Social Link. The protagonist meets her by working part-time at Inaba Municipal Hospital during evenings. Rank 4 is required to unlock the Death Social Link, Hisano Kuroda. After maxing her Social Link, she gives the protagonist her Hospital ID which allows fusion of Beelzebub.
